It is, not coincidentally, the most prescribed ED medication in the United Kingdom, and one of only two such treatments approved by NICE for daily continuous use [1].

But that versatility comes with complexity. The dose range stretches from 2.5mg at the lower end to 20mg on-demand — with generic formulations extending well beyond that — and the difference between getting it right and getting it wrong matters both clinically and practically. Too low a dose delivers little benefit; too high increases the risk of side effects without proportionate gain.

How Tadalafil Works

Tadalafil belongs to a class of drugs known as phosphodiesterase type 5 (PDE5) inhibitors. Its mechanism is well understood and extensively documented in the clinical literature.

During sexual arousal, the body releases nitric oxide (NO) into the smooth muscle tissue of the penis. This triggers the production of cyclic guanosine monophosphate (cGMP), which relaxes smooth muscle, widens blood vessels, and allows increased blood flow — the physiological basis of an erection. PDE5 is the enzyme responsible for breaking cGMP down. By inhibiting PDE5, Tadalafil allows cGMP to persist for longer, sustaining and amplifying the natural arousal response [2].

  • Tadalafil does not cause automatic erections. Sexual stimulation is an absolute requirement. The drug amplifies a natural response; it does not replace it.
  • The 36-hour window does not mean a 36-hour erection. It means the drug remains pharmacologically active for that period, enabling a natural response whenever stimulation occurs within that window.

First approved by the US FDA in 2003 and subsequently by the European Medicines Agency (EMA), Tadalafil is recommended by both NICE and the European Association of Urology (EAU) as a first-line treatment for erectile dysfunction [1][3].

On-Demand vs Daily Use: A Critical Distinction

One of the defining features of Tadalafil — and something that genuinely sets it apart from Sildenafil and Vardenafil — is its suitability for both on-demand use and daily continuous dosing. Understanding the difference is essential to choosing the right regimen.

On-Demand Use

The on-demand approach involves taking Tadalafil approximately 30 to 60 minutes before anticipated sexual activity. The drug then remains active for up to 36 hours, providing a long and flexible response window. This is the most commonly prescribed regimen for men who have sex fewer than twice weekly, or who prefer a structured, event-based approach [4].

On-demand doses typically range from 10mg — the NICE-recommended starting dose — through to 20mg for those in whom 10mg proves insufficient.

Daily Low-Dose Use

The daily regimen involves taking a low dose — most commonly 5mg, taken at the same time each day — regardless of whether sexual activity is anticipated. Over several days, a steady-state plasma concentration is established, which means the drug is always pharmacologically present. Sexual activity can occur spontaneously, without any requirement to time a dose [4].

Research published in the Journal of Sexual Medicine found that the daily 5mg regimen produced significantly greater improvements in spontaneity and relationship satisfaction compared with on-demand dosing in men who valued unplanned intimacy [5]. The EAU guidelines recommend daily dosing specifically for:

  • Men who have sexual intercourse more than twice weekly
  • Men who find on-demand dosing psychologically burdensome
  • Men with concurrent benign prostatic hyperplasia (BPH), where Tadalafil 5mg is also clinically indicated for lower urinary tract symptoms [3]

The primary trade-off is that daily dosing requires consistent adherence; missing several consecutive doses reduces plasma levels and diminishes efficacy.

Below is a clinically grounded breakdown of each available dose — what it is used for, who it suits, and what to expect.

5mg - The Daily Dose

Tadalafil 5mg is the only dose approved for daily continuous use. It is the appropriate choice for men who prefer spontaneous rather than planned sexual activity, those who have sex frequently (two or more times per week), and those experiencing BPH symptoms alongside ED.

The 5mg dose is also the recommended starting point for men over 65 or those with mild hepatic or renal impairment, where pharmacokinetics may be altered [4]. It is not suited to on-demand use, as the single dose is too low to produce a reliable acute effect.

In clinical trials comparing Tadalafil 5mg daily versus placebo, 83% of men in the treatment group reported improved erections, versus 24% in the placebo group — a difference that was statistically significant across all severity levels of erectile dysfunction [6].

Chewable (oral dispersible) tablets — Designed to be chewed rather than swallowed whole, these are absorbed more rapidly through the oral mucosa, offering a somewhat faster onset than standard tablets. They are particularly suitable for men who have difficulty swallowing conventional tablets, or who prefer a more discreet administration option. The absence of a water requirement also makes them more practical in real-world settings.

Sublingual (under-tongue) tablets — Sublingual administration dissolves the tablet beneath the tongue, delivering the active compound directly into the bloodstream via the sublingual mucosa. This bypasses the first-pass hepatic metabolism that delays the absorption of swallowed tablets, producing a notably faster onset — often within 15 to 20 minutes. For men who find a 30-minute advance window impractical, this formulation provides a meaningful clinical advantage.

40mg - Higher Strength Option

80mg - Maximum Strength Formulations

Tadalafil 80mg is the highest dose available in generic form and should be approached with particular care. It is used in very specific clinical circumstances — typically post-surgical cases involving major pelvic or urological procedures where nerve preservation was not possible, and where lower doses have been systematically tried and found ineffective over an extended period.

This is not a dose for self-titration. The cardiovascular risk profile at this level is meaningful, and appropriate screening is essential prior to use.

Who Should Not Take Tadalafil

Tadalafil is contraindicated in a number of clearly defined situations. This list is not exhaustive — a prescribing clinician will conduct a full medication and health review — but the following represent the most clinically significant contraindications:

  • Nitrate medications: The combination of Tadalafil with any nitrate medication — including GTN (glyceryl trinitrate) spray, isosorbide mononitrate, isosorbide dinitrate, and nitrate patches — can cause a severe and potentially life-threatening drop in blood pressure. This contraindication is absolute. There are no safe dose combinations [8].
  • Recreational nitrates (poppers): The same mechanism applies to amyl nitrate and butyl nitrate. Concurrent use is dangerous and must be avoided [8].
  • Recent cardiovascular events: Men who have experienced a myocardial infarction, stroke, or episode of unstable angina within the preceding six months should not use Tadalafil without explicit cardiology approval [9].
  • Severe hepatic impairment: Tadalafil is metabolised by the liver. Severe hepatic impairment significantly prolongs drug exposure and is a contraindication to use [4].
  • Hypotension: Men with resting blood pressure below 90/50 mmHg should not use Tadalafil. The vasodilatory effect of the drug can exacerbate low blood pressure [4].
  • Certain alpha-blockers: Co-administration with alpha-blockers requires caution and should always be managed by a prescriber. Additive hypotension is a recognised risk, particularly with doxazosin [4].

Common Side Effects

The side effect profile of Tadalafil is well-characterised after more than two decades of clinical use. The most commonly reported effects — typically occurring in 5 to 15% of users in controlled trials — are [7]:

  • Headache (the most common, related to vasodilation)
  • Flushing or facial redness
  • Dyspepsia (indigestion or acid reflux)
  • Nasal congestion
  • Myalgia (muscle ache) and back pain — more characteristic of Tadalafil than other PDE5 inhibitors, attributed to mild PDE11 inhibition

Serious adverse events are rare in appropriately selected patients. The most clinically significant rare effects include sudden hearing or vision loss and non-arteritic anterior ischaemic optic neuropathy (NAION). Should any sudden sensory changes occur following a dose, the drug should be discontinued immediately and emergency medical attention sought [4].

Food, Alcohol, and Timing

Unlike Sildenafil — which is significantly affected by food, particularly high-fat meals — Tadalafil absorption is not meaningfully impacted by eating. It can be taken with or without food, which contributes considerably to its ease of use in real-world settings [4].

Grapefruit and grapefruit juice should, however, be avoided. Grapefruit contains compounds that inhibit CYP3A4, the hepatic enzyme responsible for Tadalafil metabolism, which can elevate plasma concentrations and increase the risk of side effects [4].

Alcohol does not directly interact pharmacokinetically with Tadalafil, but excessive alcohol consumption independently impairs erectile function and may counteract the drug's benefits. Moderate consumption — within NHS guidelines of no more than 14 units per week — is generally considered compatible with use [9].

A Note on Generic Tadalafil and Vidalista

Vidalista is a generic Tadalafil manufactured by Centurion Laboratories, a pharmaceutical company established in Surat, India, and exporting to regulated markets globally. The branded original — Cialis, manufactured by Eli Lilly — held patent exclusivity for many years. Since that expiry, numerous generic manufacturers have produced bioequivalent formulations at substantially lower cost.

Generic medicines are required to demonstrate bioequivalence to the originator product — that is, the same active ingredient, the same dose, and an equivalent rate and extent of absorption — before market approval. The clinical effect, mechanism of action, and safety profile of generic Tadalafil are identical to branded Cialis. The only meaningful differences are cost and tablet appearance [10].
